How To Send A Reference Form
To send the reference form to your pastor, priest or other Christian leader follow these instructions.
- have his or her email address handy
- click here to see the form
- At the bottom of the form you will see a link that says 'Email this Page to a Friend'. Click on that link. A little window will pop up where you can fill in the email address of the person you are sending it to, as well as your own address so that they know who it came from.
- If you are concerned that this didn't work, you can also send them a private email the way you normally email people and explain to them what the reference form is for and give them this link: http://www.secretsofsuccess.com/info/mentor_ref.html
When they fill out the form and hit 'Submit' the information will automatically be forwarded to our mentor co-ordinator. It is not necessary to print it and send it by post.
